Our client seeks to improve the quality of life in the community through care, support, and education. Located in Roebourne, they have been delivering community services for over 42 years.
About the Role
WorkPac are currently seeking a Disability Support Co-Ordinator.
The operational hours for this position are Monday to Friday 8am-4pm with an average of 76 hours available per fortnight. A full time role will be on offer to the successful candidate.
The position comes with the use of a company vehicle, work phone and laptop.
Duties and Responsibilities
THE NDIS Support Coordinator is responsible to deliver support coordinating, support connection and life stage transition planning to participants through their NDIS plans. Their role to support the NDIS participant to achieve their plan goals, by coordinating:

- Informal supports ??? connecting with family, friends and other community groups or networks
- Mainstream supports ??? working with other sectors that have a responsibility to support NDIS participants, such as Health, Education, Housing and Justice
- Funded supports - The funding services outlined in participants??? NDIS plan, liaising with service providers who can deliver these supports
- Minimum 5 years experience in a coordination role or similar
- High level computer skills
- WA C Class Drivers Licence
- National Police Clearance
- Working with Children Check
- Minimum Cert III qualification in relevant field
